it actually shouldnt matter which order you do it in. I believe your issue is that when they refurbed your phone they resety our MSL, which is the number that you need to get into that menu. My suggestion would be to first try ##000000# if that doesn work there are a few applications that can grab the MSL from your phone; you can find these applications using the search function of the site. Once you have that you are able to do ##MSLCODE# then when you get to the menu you can continue with the MDN and MSID that they provided you with.
